2025 Annual Recertification

CHRC RESIDENTS - Important Please Read Carefully

Please be advised that it is now time to complete the 2025 Annual Recertification according to the HUD Lease Agreement and Tax Credit requirements.

Therefore, you must follow the specific instructions below and return the complete packet with your signatures no later than October 15, 2024.

Please read and follow the steps below:

  1. You will receive your Recertification packet hand-delivered to your apartment the first week in September. If you plan to use RENT CAFÉ to complete your recertification, DO NOT submit a paper copy.
  2. Please COMPLETE and SIGN all the required documents listed below:
    1. Annual Recertification Questionnaire
    2. Family Summary Sheet
    3. HUD 9887 (Required for all households & must be signed by every household member 18 & over)
    4. CHRC Applicant/Tenant Authorization for Release of Information
    5. Acknowledgement of Document Receipt by Applicant/Resident
    6. HUD 92006 Supplement to Application Resident Emergency Form
    7. THDA Verification of Student Status (Sign bottom section only)
    8. THDA under $5,000 Asset Certification (Sign bottom section only)
    9. Non-Employment Affidavit (If Resident is not employed)
    10. Self-Certification Child Support (If Resident has children)
  3. Please answer every question completely in blue ink. You must then sign and date all of the required documents included in the packet.
  4. Once you have completed all the required documents, please return your documentation to the office in the envelope provided and/or place it in the CHRC drop box.
  5. CHRC will call, email, and/or send you a letter with any questions or instructions to complete the remainder of the recertification process. 
  6. You are required to sign all your final documents including the HUD -50059 subsidy form and TIC (Tenant Income Certification). If you do not complete the 2025 Recertification process, you will be required to pay the market rent listed below for your apartment starting January 1, 2025.

  7. You must complete all the final requirements of the 2025 Annual Recertification process that includes your signature on the final documents. All paperwork must be returned and signed prior to November 15, 2024. If you have any questions, please contact us or call 931.388.5203.
